How to Claim

Forms

*Deadlines: Please review the deadlines for submitting claims as the insurance company will not accept claims past these deadlines.

IMPORTANT: To be eligible for the supplementary plans, students must have OHIP, coverage from another province, or UHIP for international students, which cover expenses such as visits to your doctor or the hospital. In some instances upfront payment will be required in advance. Additionally, there may be some out-of-pocket expenses not covered by UHIP.  For more information regarding UHIP, please visit this webpage

Change of Coverage Period

This service is provided to the majority of graduate students, however, not all students are automatically enrolled into these services. Please review the “Who’s Covered?” section on the Studentcare website to get full details. This will provide information regarding optouts, self-enrolment and couple & family enrolments.
